SUMMARY: Under the general direction of the Integrated Behavioral Health Program Director, the Case Management Specialist is responsible for supporting the mission, vision and core values of MNHC by assisting the primary care providers, Integrated Behavioral Health Therapists and other members of the health center by providing patient crisis intervention and case management services.

PRIMARY DUTIES and RESPONSIBILITIES include the following: (other duties may be assigned)

Determine the patient’s eligibility for services inclusive of placement for mental health and substance abuse patients.
Engage the patient’s participation in the assessment process including the use of information about the patient to coordinate ongoing care/treatment.
Follow-up with patients as appropriate to track and document use/success of referral.
Assumes level of expertise in the knowledge of community resources.
Facilitates referrals for patients who have lack of financial resources to support their health care needs, connecting them with resources such as the Department of Social Services, prescription support programs, etc.
Provide as needed case management and crisis intervention.
Advocate for resources to meet the patient’s needs.
Maintains an updated log of information about community resources.
Understand, respect, and adhere to patient’s rights and confidentiality.
Performs other duties incidental to the work described herein

Prepare written progress reports and submit them in a timely

Manner to the Behavior Health Program Coordinator.

QUALIFICATION REQUIREMENTS: The requirements listed below are representative of the knowledge skills, and/or ability required:

EDUCATION AND/OR EXPERIENCE: Must have at least a High School Diploma or GED. Prefer a minimum of 2 years in a medical setting.

ESSENTIAL TECHNICAL/MOTOR SKILLS: The case management specialist must possess accurate keyboarding skills. The employee must have the ability to use standard office equipment: including computers, photocopiers, and facsimile machines. He/She possesses the ability to learn, understand, and use Internet insurance eligibility systems.

INTERPERSONAL SKILLS: The Case Management Specialist must possess excellent interpersonal and customer service skills. Teamwork and communication skills are essential. The employee must treat all patients, co-workers, and managers with courtesy and respect.

MENTAL ABILITIES: The Case Management Specialist possesses cognitive reasoning skills and the ability to think clearly in high-pressure situations. The employee has the ability to prioritize and withstand pressure of continual work with variable requirements. He/She has the ability to concentrate and maintain accuracy despite frequent interruptions. The case management specialist possesses critical thinking, organizational, problem solving and reasoning skills/capabilities. The employee must be able to identify a problem, identify possible solutions, make a decision on one of the possible solutions, implement the decision, and evaluate the results.

LANGUAGE SKILLS: The employee must possess the ability to speak English fluently. The ability to speak other languages is a plus.

The employee will be working inside a multi-cultural medical/dental clinic, which delivers comprehensive health care services, including HIV/AIDS treatment to the disadvantaged poor and underserved. The building is heated in winter and air conditioned in the summer. An employee may experience noise levels of moderate to loud: children crying and screaming, loud talking patients, patients speaking multiple languages at the same time, loud speaker and/or paging systems. An employee may be subject to exposure to copier toner, correction fluid, bright fluorescent lighting, and continuous personal computer exposure.

This job is a Blood borne Pathogens risk category III position.
