The Care Coordination Specialist will report to the Vice President of Client Experience, who manages the client strategy team. The Vice President of Client Experience reports up to the Chief Operating Officer. As the Care Coordination Specialist, you will serve as an on-site resource for The Queen’s Health System in Honolulu, Hawaii.
The primary goal will be to sustain the program post go-live, assist with patient enrollment and community resource education, and overall build positive relationships within the client partner organization at the Executive, Technical, Operational, and Clinical levels. The role will be measured on KPIs tied to the clinical, operational, and financial goals of the program. A critical part of the role as outlined will include partnering with the Client Strategy Manager in driving strategy around partnership sustainability, expansion, cross-sell, and upsell opportunities and conversion of those opportunities into revenue for HRS.
Responsibilities:
- Build strategic relationships with existing clients at the Executive, Technical, Operational, and Clinical levels.
- Execute on strategy around commercial goals as it relates to The Queen’s Health System
- Serve as an escalation point externally and internally for client needs as needed
- Prepare for, in conjunction with the Client Strategy team and as appropriate based on content discussed, and lead regular business review meetings with client partners to discuss performance metrics and highlight opportunity areas
- Build client champions that become HRS evangelists
- Collaborate cross functionally with a variety of internal partners to ensure our clients are set up for success
- Monitor The Queen’s Health System EHR RPM pool and provide operational support where needed. Monitor the QHS RPM Workflows within HRS System and QHS’s EHR (Including Epic/MyChart) and identify risks and solutions
- Support enrollment process for The Queen’s Health System personnel and ensure accurate enrollment in ClinicianConnect
- Manage and track RPM inventory across all HRS use cases at QHS.
- Contact patient to arrange equipment delivery and retrieval when necessary
- Provide other administrative support activities as needed
- Provide education and support to The Queen’s Health System community resources
- Monthly report out to Client Strategy Manager and VP of Client Experience of program metrics
- Primary Quantitative Role KPIs:
- Maintaining RPM kit utilization above 60%
- CSAT scores of 90%-100%
- Patient satisfaction score of 90%-100%
Skill Requirements:
- Minimum 2 years of experience in Patient Care Coordination or Account Management (Clinical or Healthcare Experience Preferred)
- Ability to travel to in-home appointments to support community resources, including on neighboring islands.
- Ability to travel to various locations at The Queen’s Health System to support clinicians, including neighboring islands.
- Solid communication skills in all aspects (phone, email, presentations, in-person)
- Ability to communicate effectively throughout all levels of an organization, from C-suite executives to clinicians
- Intermediate Outlook, PowerPoint, Excel and Salesforce skills
- Strong skillset to work independently
- Ability to adapt quickly and prioritize effectively
- Strong and well-established organizational skills
