This position is responsible for all tasks related to the department's operations. It ensures that medical records are assembled in a standard order and are accurate and complete. It scans documents into Electronic Medical Records (EMR), processes correspondence, handles requests from patients and internal/external entities with obtaining records related to patient care, answers telephones, verifies eligibility, schedules appointments, and assists Members/requestors with questions. Responsible for safeguarding patient records and ensuring compliance with HIPAA standards and NYS regulations.
Duties And Responsibilities
- Files and scans paperwork and reports in the EMR and paper charts.
- Responds to internal/external requests for patient records, retrieving and transmitting records appropriately within the established time frame.
- Utilizes knowledge of organizational guidelines and eligibility rules when handling patient record requests.
- Ensures that legal requests are handled according to applicable laws, regulations, and organizational policy.
- Records and tracks all record requests.
- Verifies legitimacy and accuracy of all such requests.
- Escalates urgentrecords requests appropriately.
- Assists patients and internal/external entities with medical records requests and questions.
- Report PHI breaches, patient complaints, problems, or similar situations to the Team Leader/ HIM Manager.
- Follows organizational policies and procedures for onsite scheduling.
- Maintains up-to-date knowledge of Health Center policies and procedures relevant to job function.
- Performs additional clerical duties (answering phones, copying, data entry, scanning, etc.) as needed.
- Represents the Health Center to patients in a professional manner.
- Protects patients’ confidentiality and follows all applicable Federal and State regulations for safeguarding patients’ information.

- Assists in other areas and performed other duties, as needed.

Education, Skills And Experience
- 2+ years’ related work experience with medical records in a multi-specialty healthcare setting.
- Proficiency with Electronic Medical Records (Cerner preferred).
- Excellent interpersonal, customer service, organizational, verbal, and written communication skills.
- Knowledge of Microsoft Office and proficient typing skills.
- Ability to manage and effectively follow up on completing assigned duties.
- Demonstrated ability to work independently and take ownership of assigned duties and responsibilities.
- Experience in accurately using various office equipment (scanner/copier/fax).
- Strong ability to maintain positive and professional relationships with peers.
- Positive work attitude.
- High School Diploma or GED. Associate’s Degree preferred.
- Bilingual a plus (Spanish, Mandarin, Cantonese, Creole, Polish, Russian).
This role is currently located in Midtown but is scheduled to move to Brooklyn around the Fall season.
