The facility is located in Santa Fe, New Mexico, provides comprehensive healthcare services to Native American and Alaska Native communities. The facility offers a wide range of services, including primary care, dental care, mental health, and specialty services, as well as chronic disease management. Additionally, Santa Fe Client provides pharmacy, laboratory, and radiology services, focusing on preventive care and promoting overall wellness. RN Case Manager - RESPONSIBILITIES (listed are included but not limited to)
- The RN Case Manager coordinates care for ordering complex durable medical equipment and managing referrals for both un-resourced and resourced patients.
- Assist Purchase & Referred Care (PRC) staff in clarifying provider referrals/orders with non-Client vendors/providers.
- Partner with clinical care teams to ensure appropriate and timely referrals.
- Track and manage referrals entered by providers.
- Collaborate with external case managers and social workers for hospitalized patients with complex needs, including geriatric, pediatric, surgical, and trauma cases.
- Work with external vendors and partners to streamline referral processes.
- Address case management consult requests across all ambulatory clinics.
- Provide input during weekly PRC Committee meetings to support medical review members.
- RN Case Manager reviews charts, care plans, and contact patients, families, and service providers to ensure care meets patient needs.
- Assess care needs and resource utilization during consultations with healthcare providers.
- Review patient eligibility for care under applicable rules and regulations.
- Assist patients in exploring alternate resources, including Medicare, Medicaid, and private insurance options.
- Schedule medical appointments, therapies, and other services as needed.
- Revise care plans based on patient progress and input from healthcare providers.
- Collect and review health records to complete referrals and process payments.
- Ensure comprehensive and adequate patient treatment through appropriate documentation.
- RN Case Manager conducts follow-ups to confirm referral documentation and ensure patient treatment.
RN Case Manager - REQUIREMENTS - Education: Degree or diploma from an accredited registered nursing program.
- Licensure: Possess a current, valid, unrestricted nursing license in a U.S. state, the District of Columbia, Puerto Rico, or a U.S. territory.
- Experience: At least 36 months of nursing experience, including 12 months in a related specialty.
- Certifications:
- Mandatory: Current Basic Life Support (BLS) and Pediatric Advanced Life Support (PALS) certification.
- Preferred: Advanced Cardiac Life Support (ACLS), Neonatal Resuscitation Provider (NRP), Trauma Nurse Critical Care (TNCC), and Nurse Specialty certification.
